ESSENTIAL DUTIES AND RESPONSIBILITIES
<ul>
<li>Conducts required initial, quarterly, annual, and significant change assessments based on Resident Assessment Instruction (RAI) guidelines to ensure compliance with state and federal standards.</li>
<li>Collects, analyzes, and documents data about each resident’s health condition and medical history.</li>
<li>Develops, revises, and reviews person-centered care plans as necessary.</li>
<li>Communicates frequently with residents and their representatives and coordinates care plan meetings.</li>
<li>Collaborates and effectively communicates with nursing staff and facility personnel to coordinate quality care for residents.</li>
<li>Attends Staff/Unit meetings promoting, developing, and fostering a positive working environment.</li>
