JOB SUMMARY

The duties of a LMHC or LCSW include, but are not limited to, commute to multiple post-acute care facilities every day, making rounds, communicating to patients, facility staff members, providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients. Work with Psych division providers within the post-acute facilities. MUST HAVE A FLORIDA LICENSE OR BE A REGISTERED INTERN IN FLORIDA.

Monday-Friday

15-20 Patients per day

1099/W2

Flexible hours


PRIMARY FUNCTIONS

  1. Examine patients and record history
  2. Adhere to proper charting protocols
  3. Discuss with Physician and other health care professionals to prepare comprehensive plan
  4. Submit health care plan and goals of individual patients for periodic review by Physician
  5. Promote health by advising patient on diet, hygiene and prevention of disease
  6. Refer to specialist as required for patient's well-being
  7. Report deaths and contagious diseases to governmental authorities
  8. Other responsibilities as assigned

EDUCATION AND EXPERIENCE

  1. Must have and maintain current State of Florida license in good standing
  2. Must be LMHC/LSW in the State of Florida
  3. Current National Board Certification

KNOWLEDGE, SKILLS, AND ABILITIES


  1. Effective written, verbal and electronic communication skills
  2. Show initiative, creativity and problem-solving ability
  3. Ability to collaborate with colleagues to provide quality healthcare

ADDITIONAL QUALIFICATIONS

  1. Prior experience using an electronic medical records system
  2. Bilingual a plus, but not required (English/Spanish)

RELATIONSHIP REPORTING

Reports to the Supervisory Psychiatric Physician / Executive Director

PHYSICAL REQUIREMENTS

  1. Ability to communicate effectively with patients
  2. Ability to collaborate with colleagues as necessary to provide quality health care
  3. Ability to use a computer, printer and scanner
  4. Ability to sit, stand or view a computer screen for extended periods of time
  5. Ability to perform repetitive hand and wrist motions for extended periods of time
  6. Ability to use hands and fingers to handle or feel
  7. Ability to reach with hands and arms
