<p>&lt;b&gt;Location: &lt;/b&gt; REMOTE&lt;br&gt;&lt;b&gt;Salary: &lt;/b&gt;  $19.00 USD Hourly - $21.00 USD Hourly&lt;br&gt;&lt;b&gt;Description: &lt;/b&gt; Our client is currently seeking a Community Health Worker to join their team!&lt;br&gt;&lt;br&gt;Scope of Role:&lt;br&gt;&lt;br&gt;Location: Remote (Must reside in MA, CT, RI or NH)&lt;br&gt;&lt;br&gt;Hours: M-F 8am to 4:30pm&lt;br&gt;&lt;br&gt;Job Engagement: Contract &lt;br&gt;&lt;br&gt;Ideal candidate has some community health work background or experience in related field such as, case management, coordinator, or medical assistant.&lt;br&gt;&lt;br&gt;&lt;b&gt;This job will have the following responsibilities:&lt;/b&gt;&lt;ul&gt; &lt;li&gt;Completing Welcome Calls for members who are new to the plan&lt;/li&gt;&lt;li&gt;Document all telephonic encounters/ attempts in medical records system&lt;/li&gt;&lt;li&gt;Provide benefits information to new members and answering benefits related questions&lt;/li&gt;&lt;li&gt;Confirm member demographic information and request updates&lt;/li&gt;&lt;/ul&gt; &lt;b&gt;Qualifications &amp; Requirements:&lt;/b&gt; &lt;ul&gt; &lt;li&gt;High School Diploma or GED&lt;/li&gt;&lt;li&gt;Previous experience as a medical assistant, care coordinator or case management.&lt;/li&gt; &lt;/ul&gt;Interested?
